Physician Responded Update on brain dead brother

1.5k Upvotes

Hello. I’m the sister of the 15 y/o boy (who almost completed 16) who was in a devastating accident in December 2025 that lead to his brain death within the first week. After almost eleven months of my family suffering to keep him on life support despite knowing that he’s never waking up, he was finally able to rest in peace yesterday afternoon. His heart stopped on its own after ten hours of spiralling and multi-organ failure.

My heart is now at ease, and many thanks to all the medical personnel on this sub and everyone who sent me condolences and prayers, this means much more to me than anyone realises.

Thank you all for your support, genuinely, and I pray that none of you are ever put in this horrendous situation 🤍

Physician Responded My 56F mother fell down the stairs, GCS 3, on ventilator, fixed pupils. Seeking advice on her CT scan.

Enable HLS to view with audio, or disable this notification

1.1k Upvotes

My mother fell down the stairs on October 24, 2026, at around 00:00 (UTC+8) and hit the back of her head. There was bleeding, and she still hasn't woken up. A CT scan was done. She is currently on a ventilator and vasopressors, her Glasgow Coma Scale score is 3, and her pupils show no reaction to light. (She drinks alcohol every day and previously took psychiatric medication. Could that affect the pupillary light reflex?)
Additional information:
Age: 56
Sex: Female
Height/weight: approximately 165 cm (5'5") / 57 kg (126 lb)
Smoking: Non-smoker
Alcohol: Drinks alcohol every day (exact amount unknown)
Medications: She previously took sleeping pills and antidepressants, but she is not taking any psychiatric medication now that she is in the ICU. I don't know the exact names.
Other medical history: Unknown
Current condition: Unconscious since the fall, on a ventilator and vasopressors, GCS 3, no pupillary response to light.
I really want my mom to get better, and I feel so desperate. Could anyone who looks at the CT offer some advice?
English is not my native language, and I am using translation, so please forgive me if anything reads awkwardly.

Physician Responded Spot on hand 10 days post c-section

Post image
53 Upvotes

I am 32F, and had a baby via emergency c-section 10 days ago. Today this spot appeared suddenly on my hand under the skin in the span of 20 minutes or less (we were on our way home from my son’s pediatrician, I washed my hands right before we left - no spot then - and it showed up on our way home). I didn’t hit it on anything or grip anything that might have done this. I normally wouldn’t stress but the postpartum hormones have me a little anxious and my OB hasn’t returned my call yet. Is this anything I should be concerned about?

Other info:
32F, 5’4”, probably around 190lbs at the moment. Meds I’m taking include 20mg Citalopram daily, prenatal vitamin, and magnesium glycinate. I’m not breastfeeding.

Physician Responded 37F Pupil still dilated 3 weeks after fall… blurred near vision

Post image
42 Upvotes

I am a 37F, 115lbs, 5’5”. I have a seizure disorder and take Topiramate, Lamotrigine, and Gabapentin.

10/6 I suffered a seizure, fell and hit my head on a sidewalk. Airlifted to trauma center when they noticed one pupil was dilated and not reacting to light. CT and CTA came back normal. I was discharged a few days later saying it was likely a symptom of the concussion or possibly from an unknown medication.

Fast forward 3 weeks later and the pupil is still dilated. No longer seeing double, but my close up vision in that eye is blurry. It is also more sensitive to light than the other one.

Should I be following up with an eye doctor or something? There were no instructions given to me on that and I’m starting to get a little worried about it.

Thank you for your help!

From highly productive person to slob

I’m a rather well-accomplished person and have a very lucrative job. I am also young in my early twenties. I am capable of being very productive.

Over the last month my apartment has become a cesspool genuinely disgusting I haven’t picked up a single thing there’s rotten food and flies. I’m not showering or brushing my teeth. I have to go to work every couple days so I’ve been buying new clothes (all my clothes are dirty) and ordering food to my door. I eat what I’m want and let the rest spoil. Can’t sleep on my bed because too much shit on it so I sleep elsewhere. I’m literally disgusted with myself.

Im also a super social person so do this mad hygienic effort when I go out to see my friends like everything shower and buy new clothes. I literally am flabbergasted with myself right now and to make matters worse I live on a super busy street so I have to keep my blinds closed or else people would see the mess.

I also just feel unwell. I’m worried something is going on with me. I am certainly not depressed. I’m happy! Just a suddenly messy woman I think? I want to know what I should do and if this sounds like it could be a medical problem?

My life has fallen apart overnight and we have no idea why. Please help me.

26F 5'2", 247lbs.

Diclofenac, Latuda, Lamictal, Klonopin, Baclofen, Lyrica, Buspar, Ramelaton, Duloxetine, Metformin, Addiy. Benedryl every 4hrs.

I am aware I am on a lot of medication, I have done numerous medication reviews, I cannot change anything currently even if I wanted to before we do the EEG. Meds have not been adjusted whatsoever in half a year asides from benedryl.

HSD, SI Joint dysfunction, Nerve pain, BPD, PTSD, chronic long-term constipation.

It started after I got very very sick (like piss myself at work sick) two months ago. It was confirmed not COVID or the flu. After that, I began experiencing migraines for the first time in my life. They're in a different place every time, ranging from pressure behind my eyes, to what almost feels like a nauseating visceral brain freeze. They are not localized in one area or one side of my head. Benedryl stops these, Nurtec does nothing, Dramamine does nothing

Then I began experiencing extreme dizziness. If I'm awake longer than 5hrs at a time, everything becomes static-y, especially when turning. Like you've stood up without breathing. My vision will blink out and I will begin to black out and cannot keep my balance. If I wait too long to sleep, this then turns into feeling as though the room is spinning on its axis, worse when my eyes are closed. Diclofenac helps with this.

Short of breath constantly but o2 stats fine. If I go too long without the Diclofenac, everything in my face becomes severely inflamed- my sinuses, the roof of my mouth, my jaw, my ears become hard to pop and painful. It constantly feels like I'm hanging upside down, and the blood is pooling to my head, worse when bending down and standing up. I cannot do any activities that require bending down and standing up without risking passing out.severe pressure and pain in my eyes.

Eyes around iris and eyelid grow puffy only at night, with or without eye drops, with or without benedryl.

I cannot hear well. I can tell there's sound, but I can't interpret it unless it's louder than normal. I've had to turn the volume up on every device I use for the first time.

Weight briefly went up 7lbs in 8 days, no edema, but has gone back down and stabilized.

Any type of outing, or even just getting dressed, leaves me wiped and extremely fatigued for several days after. I have been unable to leave my apartment with one exception for weeks.

Heart palpitations.

I've started experiencing severe night terrors where I will thrash my head/shake and whimper at night. My fiance has to wake me up.

I have started, for the first time, experiencing flashbacks very vividly. For the first time also, I can picture faces. My pets, and my partner.

I have severe strabismus. Lately I've found it incredibly easy to pull my eyes together, and I can see more in 3D than normal.

My hallucinations came back in a weird way. I heard my fiance's ringtone for 3 days straight, and kept seeing black spots on the wall in the corner of my eyes. I began eating with the Latuda, and it has since thankfully ceased.

I've started experiencing severe acid reflux.

I've stopped eating and primarily drink sugary drinks.

My hands have begun twitching uncontrollably at night and I find myself with my jaw hinged to the right when at rest, which pops it out of place. I crunch it back.

Due to the severeness of these symptoms, I have been out of work for a month on medical leave. I cannot afford to miss more work. I have to use a shower chair due to being a fall risk.

I've been to the ER 3x, the walk in 2x, and my PCP 3x, Neurologist once, opthamologist once. EKG normal, CT scan and MRI w/o contrast of brain normal, ears grossly clear. Basic metabolic labs normal asides from increased WBC. We have a lumbar puncture on the 11th to rule out IHH, I have to schedule an EEG to rule out seizures (we deem this unlikely) and I have trypase (?) labs waiting for me to complete to rule out MCAS. Opthamology showed normal, no swelling of optic nerve or inner eye.

Please help me. I want my life back. I want to shower independently. I want to go back to the job I love. I want to stop relying on my partner so much. I want the appointments to stop. I am so, so tired. I just need an answer so we can fix this somehow. Please, if anyone has any ideas, please help.

Thank you so much

Physician Responded 17F, I swallowed a small sharp piece of metal, maybe multiple small pieces. Will I be okay?

context: my blender started making some funny noises, but i ignored it and downed my protein shake. at the bottom of the glass, i saw multiple small metallic pieces shining in the light. am i going to die...??

the pieces were half a finger in length and the thickness of the metal shavings you get when you sharpen a pencil. they had pointy ends.

i can't talk to my parents about it because i'm afraid they'll just panic/scold me for being irresponsible.

edit: i now feel discomfort in the lower left section of my abdomen. i am not sure if i am imagining the pain and overreacting. help-

Physician Responded How to change my life?

I am 24F, I weigh 271.

My issues:
Type 2 diabetes, hypertension, vitamin D deficiency, fatty liver, gallstones, asthma, pcos, lactose intolerance, depression, anxiety, problems with constipation and diarrhea and now recently on the cusp of high cholesterol. Also my mom died from colon cancer so they want me screening when I’m 30 yippie!!!

My meds:
Ozempic, lexapro, abilify, amlodipine, vitamin D, Metamucil daily and occasionally miralax and insulin

Active plan:
For about 3 months I’ve been prioritizing diet and exercise. I’ve been experimenting with healthier food options that I enjoy and so far it’s been going okay.

Now here’s my dilemma, I’m overwhelmed in my issues cuz I feel like I got it all. My diabetes is out of control with A1C of 12. So I’ve been trying my best to eat good and cut back on sugars.

But it’s so hard. Like how can you motivate ur patients to be committed? Most of my doctors just say lose weight and you’ll be fine but never really explain the long term affects of all of these problems. All they tell me is that imma die when I’m 40 and I don’t want that lol. I know they’re being realistic though.

I’m trying to lose weight but it’s been going super slow even with the Ozempic so like what am I doing wrong 😅? 5 years ago I was at my highest of 245 and got it down to 220. I moved to a new city, shit happened and then I was 285. I got it down to 271 thanks to the Ozempic and lifestyle changes but like, I wanna be 200 and less like yesterday lol.

What are diets you would reccomend for me? I’ve seen a lot of different stuff on the internet. I feel like what works for one diet doesn’t work for the other. So I’m just kind of overwhelmed and feel like all I should eat is ice water soup daily.

Special note: plz be nice cuz like I am very aware of my issues 😢 I just wanna get pointed on the right direction with dieting and expectations of managing diabetes, hypertension, and fatty liver.

How badly have I messed up my feet from walking?

I am 23F, 170cm 66kg and I've had an obsession with walking in circles since I was a little kid. I think it has something to do with my autism but it just helps me feel better when I'm anxious.

How the walking mainly manifests is in my room in circles for hours on end while listening to music and thinking about different stuff. It's been a mainly harmless habit for most of my life and even helps keep me sane since I struggle with major depressive disorder that's on the level of psychosis when it gets bad. Most of the depression stems from C-PTSD since I was severely abused and neglected most of childhood. The walking helped me cope a lot as a child and it still does.

The problem is that I've started walking even more than usual and I'm scared I'm going to actually damage my feet. I usually average 15 to 25 thousand steps in a better place mentally but after an exceptionally traumatic experience two years ago my mental health has been worse than ever. I was so much better two years ago but after that incident the depressive episodes have come with paranoia and mild auditory hallucinations.

I'm now on one antipsychotic and going to therapy and really don't want more medications. I've been coping with the sadness by walking and now I'm averaging 30 to 50 thousand steps a day. My high score is over 80 thousand. A few months ago before this episode the walking had never hurt my legs. Probably because I'm so used to it but now my feet hurt like hell. The entire bottom of my foot has a dull ache. The toe pad part on my right foot developed a painful corn that I ripped out myself but now when I step wrong there's a sharp pain. I have a few places in my foot where i get random pains when walking. My heel hurts and my calves feel sore and painful as well.

Walking overall hurts but I can't stop. It's the only thing helping me stay in one piece and I need the walking. I'm just wondering if this is something my feet will get used to in a few weeks or I'm actually going to hurt myself and then not have the option of walking for a while. I genuinely don't know how to cope with that but I also can't really slow down the walking. Especially since I keep waking up from nightmares and the walking helps me calm down when I wake up multiple times a night in a state of fear.

I want to know if in a few weeks time I'll be fucked or if my body will most likely adapt? Also how can I reduce the risk of injuring myself to the point I can't hold anymore weight on my feet? Right now the pain is something I can walk off so to speak. What can I do to buy time?

My boyfriend spends all day in the gym and is becoming gaunt

My boyfriend just turned 20, male, about 5’11”, with no medical conditions. No medicines but he takes lots of vitamins and supplements.

I can see his ribs protruding when he takes his shirt off. He doesn’t eat enough food. Boiled chicken is all he eats sometimes vegetables but he takes a lot of supplements. He’s miserable. He has muscle but it’s like he’s an extremely shrunken body builder all his shirts hang off him and they are size medium only and used to fit. I think he’s obsessed. He wanted a better taper. I think that’s small waist big shoulder but I just don’t get it he looked fine and already looked good. He shouted at me when I told him I’m worried. It’s not his usual self he is irritable now.

Camera roll is him shirtless and sad. He spends ages filming himself exercising watches back shaking his head and deleting the video. I have seen him spend half an go hour staring at himself in the mirror. Its not in a vain way he looks so miserable like he can’t look away.

I’m very worried now because yesterday he fainted when walking up the stairs. I am so unqualified and inexperienced to help him and I don’t know what to do whether him fainting means it’s really bad or if he needs urgent or non urgent help. Today he’s been in bed all day. We were both supposed to be at uni today but I was too scared to leave him alone. I really don’t know what to do. I need help to make the right decisions. I think he has an eating disorder but I don’t know how to help.

Physician Responded Ruptured brain aneurysm with subarachnoid hemorrhage

Hello,

I’m posting here to get some opinions about my mother’s situation. She is 76 years old. A few days ago, she suffered a ruptured brain aneurysm, probably during the night. She lives with me, and I found her several hours later. She is currently hospitalized in a public hospital and is in a coma under sedation.

The medical team (neurologists, neurosurgeons, etc.) has given us a rather pessimistic outlook. The aneurysm is located on an artery at the back of the brain, in a very delicate area. At this stage, no surgery is planned because of its location and the associated risks.

My mother is a light smoker, is being treated for high blood pressure, takes blood thinners, and is also on a few other less significant medications.

Based on your knowledge or experience, are there any options, even rare ones or available at other specialized centers, that could potentially give her a chance of waking up? Or, in this type of case, is the prognosis generally as bleak as we have been told?

I can provide more details (specific medications, medical reports if I have access to them, etc.) if that could be helpful.

Thank you in advance for your replies.

Physician Responded Do these stitches look normal? Black/purple skin.

Male
10 years
55 pounds
No medications
Previous tonsillectomy

He jumped off the counter yesterday and landed on the drawer—tore open the back of his thigh. The wound was very deep, and you could see fat etc. Minimal bleeding. It was flapped open, as I’m sure you can imagine from the shape.

No bleeding or oozing since. Changed the dressing tonight (about 36 hours post-injury) and noticed the dark tissue about the stitches. Almost looks purple/black. Figured it was bruising, iodine, some blood, but very uncertain as I have never dealt with this before. No extreme pain. Has taken Advil twice but other than that, is acting normal, has no fever, and apart from some expected pain.

Should he be seen promptly or is this normal healing?

Thanks!

Out of breath doing nothing

29, Female, 5ft tall, 100 pounds, taking birth control (YAZ generic) and spironolactone for hormonal cystic acne and heavy periods.

For the past 2-3 days, plus today, I’ve been out of breath just sitting down. It’s extremely difficult to take a deep, satisfying breath. Sometimes I can get one in if I yawn. But it just feels like my lungs can’t expand enough and each breath keeps getting cut short because my ribcage is too small or something. When it gets really bad, it feels like there’s a weight on my chest and it just feels very constricted. I’d have barely eaten anything at all and I will feel like this. I will be having hunger pangs but can’t eat because I can’t breathe. I don’t know if this is an emergency or not but since I don’t have insurance I’d rather not go to the ER if it’s not life threatening. One of my legs had gotten really sore, like I had worked out on it but I didn’t. I’m completely sedentary. Any ideas?

Physician Responded 40m, epileptic, having unusual breakthrough seizures along with other concerning symptoms. Not sure if they are related to epilepsy or not. ED, or wait another month for neurologist appointment?

40m
6’4
210ish lbs
Epilepsy, major depression, generalized anxiety, OCD, thrombocytopenia, GERD, hypertension

1.5g keppra BID
20mg pantoprazole morning
60mg duloxetine morning
30mg lisinopril morning
50mg hydroxyzine PRN q8
75mg trazodone as needed go sleep

This is going to be difficult for me to describe because much of it I have no memory of.

Recently I’ve been having breakthrough seizures and different kinds of seizures that I typically don’t have. I usually have tonic clonic while I’m sleeping, and sometimes I have focal aware and focal loss of awareness seizures. I have had some myoclonic seizures that I remember, but this is not usual for me.

In the last 24-48 hours I’ve had at least 3 tonic clonic seizures, and possibly a new kind.

Last night my dad said that I was completely out of it, trying to say something but couldn’t get any words out. He suggested that I write it down, but apparently I would pick up the pen, lean over towards the paper, then just like, slide out of consciousness for a minute or two, without falling down.

Then I apparently went and sat on the back of the couch for about 10 minutes, mumbling to myself unintelligibly.

Then I apparently went to my room (which he said took a good 5 minutes even though it was maybe 15 feet down the hall). Then I apparently just collapsed in my bed and essentially immediately fell asleep.

I definitely had a TC last night, I can always feel when I’ve had one. I get incredibly fatigued and sore, often bite my tongue. My father did not witness the TC.

Recently I’ve been having these spells where I feel like I’m dreaming and on the brink of losing consciousness. I remember them similarly to how you remember a dream that’s slipping away.

My question is, should I wait until 18 November for my scheduled neurologist appointment (Mayo Clinic) first time I’ve ever seen him. Or should I go to the ED?

My fear with going to ED is that I’ll essentially be wasting the time of the nurses and doctors and they will basically just tell me there’s nothing they can do and to just wait for my upcoming appointment.

Thanks in advance for answering.

Physician Responded Removal of medical record that taints every interactions I have, and keeps me from visiting the doctor.

33, 6' 3", 200lbs, male, no medications (part of the issue!), don't smoke and drink maybe twice a year, current medical issues are all physical and will be discussed, eastern Washington.

Around 12 years ago (age 21) I was given a small piece of foil at a party by an acquaintance at the time. it contained fentanyl. My partner and I were very naive and new to partying. we had attended just a few parties, but we're friends with some party monsters. When this was given to me, the effects were described as "like Ecstasy" and I was told to smoke it with a straw. At the time we were young stoners, and I didn't think much of it. one morning I decided to try it and ended up waking up with my head in a litterbox, surrounded by 20 medical and fire personnel. No idea what had happened, but I had come very close to death and was brought back by narcan.

I was getting ready to take a shower, so I was totally naked and before I was rushed away in a foil blanket I only had time to put my underwear on.

Before I could get my bearings, I was in the back of an ambulance on my way to the hospital. By the time I realized what was going on and asked them to stop I was told by the paramedic that if I tried to leave they would call the police, leave me, and have me arrested. I hadn't even had the chance to say no to the ambulance ride to begin with.

As previously stated, I had extremely little experience with drugs. Besides trying MDMA and weed, I didn't really know what I was doing. That said, I was scared to death of being dropped on the side of the road nearly naked and having the police show up and testing me for drugs.

When we got to the hospital I was totally honest and told them everything.

This led to me overhearing a woman in the hall tell my fiance to leave me (which happened promptly after), my picture ending up circulating on local Facebook communities which was a different level of embarrassment, and most importantly permanently tarnishing my medical records.

After the event, I committed to total sobriety until recent years. I'll occasionally smoke or have a drink. The issue falls on going to the doctor.

I have a few major medical issues I want to address, but my experience in recent years has been horrible.

The most recent procedure I've tried to have (mind you, because of all of this I avoid medical institutions like the plague) was a very minor surgery on a digit. It should have been cut and dry, but I had the anesthesiologist grill me for 10 minutes on my drug use. It got to the point where I told him he could either give me a drug test and stop probing me after he already had sufficiently or I was going to leave. Unbelievably uncomfortable and I'm at the place I'd rather die than deal with another asshole giving me the third degree because of a naive move when I was younger. I'll spare you the rest, but it has happened two times prior to that.

I wish I could take a test or something to just remove this from my file. It feels like an insult to me embracing sobriety, and when I've said that I have gotten eye rolls from MEDICAL PROFESSIONALS.

You can't imagine the waves this made in my life 10 years ago. I lost my partner, my home, multiple friends because my partner told people and now I find I can't even use the healthcare system. The embarrassment is immeasurable. I feel so much shame because of a tiny piece of foil I was handed at a random party. To this day I have a family member who won't speak to me because of the relationship they had with my partner and assumptions made. I never even had the ability to explain myself.

Sorry for the rant. My question is; how do I remove this from my record? I would do anything to do it. I need medical care but I can't deal with the embarrassment anymore.

The profound effects this has had on my life have caused severe damage to my self-worth, relationships and everything connected. Is there anything I can do? if you have any other questions I'll be glad to answer. Thanks for reading.

Edit: Thanks for the thoughtful replies.

Physician Responded Heart Thumping Sound When I Open Mouth- Cardiologists Please Watch/Answer to Save a Patient!

Enable HLS to view with audio, or disable this notification

ASKING A CARDIOLOGIST or VASCULAR DOC- So I have had heart issues and had a cardiac stent placed in my proximal LAD for a 99% stenosis 2 years ago (got it before a heart attack thank God and I live to tell the tale).

Very Young Male (30’s), no previous heart issues, college athlete and was otherwise healthy (no smoking no drugs or alcohol) and came out of nowhere and doctors still have no idea why it happened (chalked up to genetics, all arteries are completely clean otherwise than that tiny area proximal LAD).

Ever since the stent placement- when I get my heart rate higher (120+ during/after working out, etc) i can open my mouth and I hear my heart pumping HARD (listen to video volume up). We have showed this to cardiologists and they have no idea what it is and they think it’s very odd.

Mind you- ever since the cardiac stent was placed- I have had continuous left sided (T4 distribution to left shoulder blade) pain. Cardiac MRI shows no inflammation anywhere, had a cath- showed stent looked “beautifully endotheliazed no inflammation in stent on OCT, completely patent”. I have been searching high and low for answers for 2 years+ on my issues. Been to the Cleveland clinic, and over 5 cardiologists. Somebody needs to know what this is as a clue, right? PLEASE HELP!

I blurred out my face via AI just to help conceal my identity. Thanks for all the help! Anything further please ask- my chart is insane and I have been through every test and workup possible- pulmonary, cardiac, nerve- pain management, endoscopy, allergy, rheumatology, hematology, you name it.

I am allergic to nickel we found out- not sure if that has relevance. Cardiologists say no- but we believe I may be allergic to this coronary stent and somehow some way I need it removed.

My quality of life is bad. The pain is bad, and we have no idea what to do next other than find a surgeon to remove it somehow because it saved my life- but at a huge cost.
